目的：分析肺结核患者住院费用构成及影响因素情况，为减轻患者疾病经济负担提供参考。方法：选取广西南宁市某结核病专科医院于2018年1月至2019年8月诊治的5328例肺结核住院患者，采取描述性分析、非参数检验、多元线性回归等方法对住院费用的影响因素进行分析。结果：肺结核患者住院费用中位数（四分位数）为11939.02（6911.64，19933.65）元。在住院费用构成中，诊断费（37.0%）和药品费（26.7%）占比最高。患者性别、年龄、医疗付费方式、手术情况、住院天数、入院疾病诊断数对住院费用的影响差异均具有统计学意义（均P＜0.05）；经多元线性回归分析结果显示，入院疾病诊断数和住院天数是住院费用的主要影响因素。结论：优化临床路径，适当加大医疗减免力度，有效缩短住院天数，是降低住院患者医疗经济负担的关键。
Abstract:
Objective: To analyze the composition of hospitalization expenses of pulmonary tuberculosis patients and the influencing factors, so as to provide reference for reducing the disease economic burden of patients. Methods: 5328 hospitalized patients with pulmonary tuberculosis diagnosed and treated in a tuberculosis hospital in Nanning city from January 2018 to August 2019 were selected. Descriptive analysis, non parametric test, multiple linear regression and other methods were used to analyze the influencing factors of hospitalization expenses. Results: The median (quartile) hospitalization expenses of tuberculosis patients was 11939.02 (6911.64,19933.65) yuan. In the constitution of hospitalization expenses, diagnosis expenses (37.0%) and drug expenses (26.7%) accounted for the highest proportion. Gender, age, medical payment method, operation situation, days of hospitalization and the number of hospitalized disease diagnose had statistically significant differences in the impact of hospitalization expenses (all P＜0.05) the results of multiple linear regression analysis showed that the number of hospitalized disease diagnose and days of hospitalization were the main influencing factors of hospitalization expenses. Conclusion: The key to reduce the medical economic burden of hospitalized patients is to optimize the clinical path, appropriately increase the intensity of medical relief, and effectively shorten the length of hospitalization.
