摘要:
目的：观察小针刀整体松解结合康复疗法治疗膝骨性关节炎的临床疗效。方法：选取 2016 年 6 至 2019 年 1 月北海市中医医院骨伤科门诊就诊的 60 例膝骨关节患者，按随机数字表法随机分为治疗组及对照组各 30 例。治疗组予小针刀配合康复疗法治疗，针刀整体松解术每周 1 次，3 次为一疗程，余康复项目按康复疗法操作进行；对照组单纯予玻璃酸钠注射液注入膝关节腔内治疗，每 7 天注射 1 次，5 次为一个疗程。结果：与治疗前相比较，治疗 2 周、1 个月、3 个月后，2 组患者疼痛明显缓解、膝关节屈伸能力明显好转、晨僵时间减少，VAS 评分、WOMAC 评分均明显下降（P＜0.05），且同一时间点治疗组 VAS 评分、WOMAC 评分均低于对照组（P＜0.05）。同时在疗效方面比较，治疗组显效 26 例，有效 3 例，无效 1 例，对照组显效 5 例，有效 22 例，无效 3 例，治疗组显效率 86.67%，远高于对照组 16.67%的显效率，表明治疗组疗效优于对照组（P＜0.01）。结论：小针刀整体松解结合康复疗法治疗膝骨性关节炎可有效缓解患者疼痛，改善患者膝关节功能，值得临床推广。
Abstract:
Objective: To observe the clinical effect of small needle knife combined with rehabilitation therapy for knee osteoarthritis.Methods: From June 2016 to January 2019, 60 patients with osteoarthritis who visited the orthopedics department of Beihai Hospital ofTraditional Chinese Medicine were randomly divided into the treatment group and the control group according to the random number tablemethod, 30 cases in each group. The treatment group was treated with small needle knife combined with rehabilitation therapy, the wholeneedle knife release was once a week, 3 times as a course of treatment, the remaining rehabilitation items were operated according torehabilitation therapy; the control group was treated with sodium hyaluronate injection alone, once a week, 5 times as a course of treatment.Results: Compared with before treatment, after 2 weeks, 1 month and 3 months of treatment, the pain of the two groups was alleviated, theflexion and extension of knee joint improved significantly, the morning stiffness time was decreased, VAS score and WOMAC scoredecreased significantly (P<0.05), and the VAS score and WOMAC score of the treatment group at the same time point were lower thanthose of the control group (0.05). At the same time, compared with the control group, the treatment group showed significant effect in 26cases, effective in 3 cases, ineffective in 1 case, effective in 5 cases, effective in 22 cases, ineffective in 3 cases. The treatment groupshowed significant efficiency in 86.67%, far higher than the control group in 16.67%, indicating that the treatment group was better than thecontrol group (P<0.01). Conclusion: The treatment of knee osteoarthritis with small needle knife and rehabilitation therapy can effectivelyrelieve pain and improve the function of knee joint of patients, which is worthy of clinical promotion.
