摘要:
目的: 探讨中药汤剂大承气汤保留灌肠对胰十二指肠切除术后胃肠功能恢复的临床疗效。方法：将67 例行胰十二指肠切除术的患者随机分为两组，治疗组（36 例）采用大承气汤保留灌肠，对照组（31 例）采用常规治疗，观察并记录两组患者肠鸣音恢复时间、肛门排气及排便时间，检测病人术前、术后第1 和第2 天血清胃动素（MOT）。结果：治疗组出现肠鸣音、肛门排气及排便恢复的时间较对照组明显缩短（P＜0.05），两组病人术前MOT 值无差异，术后第1d、2d MOT 上升值治疗组明显高于对照组（P＜0.05）。结论: 中药汤剂大承气汤保留灌肠能够促进胰十二指肠术后胃肠功能恢复，临床效果好，安全性高，值得推广。
Abstract:
Objective: To exploer the clinical effect of retention enema with Dachengqi Decoction on gastrointestinal function recovery after pancreaticoduodenectomy. Methods: 67 patients undergoing pancreaticoduodenectomy were randomly divided into two groups. The treatment group (36 cases) received retention enema with Dachengqi Decoction and the control group (31 cases) received routine treatment. The recovery time of bowel sounds, the time of anal exhaust and defecation were observed and recorded, and the MOT of patients before operation and on the 1st and 2nd day after operation were detected. Results: The recovery time of bowel sounds, anal exhaust and defecation in the treatment group was significantly shorter than that in the control group (P＜0.05). There was no difference in preoperative MOT values between the two groups. The increase of MOT in the treatment group was significantly higher than that in the control group on the 1st and 2nd day after operation (P＜0.05). Conclusion: Dachengqi Decoction retention enema can promote the recovery of gastrointestinal function after pancreaticoduodenectomy, with good clinical effect and high safety, which is worthy of promotion.
