摘要:
类风湿关节炎（Rheumatoid Arthritis，RA）是影响多个关节的全身免疫性疾病，膝关节最常受累。早期诊断和及时治疗尤为重要，当抗风湿药物无法控制疾病时，外科手术是必需的。目前对于关节镜滑膜切除术的手术时机选择、治疗效果仍存在一定争议。关节镜滑膜切除术通过多入路清理关节腔，使得早中期膝类风湿关节炎患者有效地缓解疼痛、改善功能，具有创伤小、恢复快等优点，并且早期手术效果优于中晚期。关节镜滑膜切除术是一种相对安全的方法，但术后仍存在放射学进展，以及复发率较高等问题。文章通过对近几年国内外文献资料的回顾，针对镜下滑膜切除术的作用、手术时机、手术方法、治疗效果等方面予以综述,为临床手术治疗早中期膝类风湿关节炎提供参考。
Abstract:
Rheumatoid arthritis (RA) is a systemic immune disease affecting multiple joints, and the knee joint is most often involved.Early diagnosis and timely treatment are particularly important. Surgery is necessary when anti-rheumatic drugs cannot control the disease.At present, there are still some controversies about the timing and therapeutic effect of arthroscopic synovectomy. Arthroscopicsynovectomy clears articular cavity through multiple approaches, which can effectively relieve pain and improve function in patients withknee rheumatoid arthritis in in the early and metaphase stages. It has the advantages of less trauma and faster recovery, and the effect ofearly operation is better than that of middle and late stage.It has the advantages of less trauma and faster recovery, and the effect of earlyoperation is better than that of middle and late stage. Arthroscopic synovectomy is a relatively safe method, but there are still someproblems such as radiological progress and high recurrence rate. This article reviews the literatures at home and abroad in recent years, andsummarizes the role, timing, methods and therapeutic effects of endoscopic synovectomy, so as to provide references for clinical operationin the treatment of early and metaphase stages knee rheumatoid arthritis.
